Wellness Questionnaire Soccer Ball

Resources Needed: soft ball like a soccer ball or beach ball, marker to write on the ball with, and some questions related to the health and wellness lessons

Instructions for Play: Have all the participants stand in a circle and pass the ball back and forth to each other. When a participant catches the ball they are responsible for the pentagon shape that their right thumb lands on. If it is a question, they answer it. However if they landed on a black shape then they get to lead the class in some exercise or physical activity. You could cap this for one minute or a certain number of repetitions of the exercise.   This can count as some of your group’s daily activity and also double as a self-esteem and confidence building exercise.

Instructions for Construction: On a soft soccer ball write health and wellness questions in each of the pentagon shapes. Some examples include:

Q. What is a good way to make sure you eat your veggies?
A. Add lettuce, tomato, and onions to your sandwich; eat a salad with your dinner, etc.

Q. Food grown without pesticides or chemicals is called what?
A. Organic.

Q. Fill in the blank: For good nutrition, focus on _____ and _____.
A. Fruits; vegetables.

Q. How do you reward yourself for making healthy choices?
A. Answer will vary.

Q. What should you always do before you exercise?
A. Warm-up.

Q. Should you take the stairs of the elevator?
A. The stairs.

Q. What food group is rich in Vitamin C?
A. Fruits.

Q. Name five healthy fruits.
A. Answer will vary.

Q. How many minutes a day should you exercise?
A. At least 30 minutes of moderate activity.

Q. How long do you hold a stretch?
A. 10-15 seconds.

Q. Legumes like beans, peas, and lentils are vegetables with lots of muscle building _____.
A. Protein.

Q. What is the healthiest part of the chicken to eat?
A. The breast because it is lean white meat.

Q. What is a good source of healthy oils and fats?
A. Fish, avocado, olive oil, etc.

Q. What is your favorite outdoor activity?
A. Answer will vary.

Q. What kind of grain is best?
A. Whole grain.

Q. Name five healthy snack options.
A. Answer will vary.

Q. What foods are a good source of calcium?
A. Dairy, milk, cheese, yogurt, etc.

Q. Dancing, walking, or riding your bike are good examples of what kind of activity? And why is that good?
A. Aerobic activity; provides good exercise for your heart.

Q. What is your favorite healthy food?
A. Answer will vary.

Q. What motivates you to be healthy?
A. Answer will vary.

Q. What is your biggest challenge for making healthy choices?
A. Answer will vary.

Q. What is your favorite thing about yourself?
A. Answer will vary.

Q. Why is it important to drink water?
A. Our bodies are made up mostly of water, to prevent dehydration, water has no calories or sugars, etc.

Q. What is your favorite physical activity?
A. Answer will vary.

Alternate Ideas: You could also use a beach ball or a ball without any lines on it and draw straight line shapes to write your questions and physical activity cues on it.
